Its a legitimate concern. Luckily, with antibiotics we generally see efficacy quickly at the individual level. Unlike something like cancer prevention, where it takes years of study in large populations.
Lime disease is a little different as there's a chronic aspect of it too.
As an aside, if you're referring to the book "Turtles all the way down" I read that and gave a review of it to some friends. It makes interesting points but interestingly cedes the point that existing vaccine studies can be taken at face value for efficacy. Its main argument was against safety, since the "placebo" studies included preservatives and other excipients. I dont believe this was malice, but was done to preserve "equal treatment" between study and control groups. Again, this is useful for efficacy but not for safety if you want to prove the safety of those ingredients in both arms.

Oh I def didnt mean doxy is the gold standard, just that that you can't get research approved or a drug to market on placebo controlled trials when there is standard of care established.
With that being said, the evidence I've seen in the past was specific to MRSA, acne, and malaria. I have not personally looked into efficacy against lime.

Its better to get superiority comparison than non-inferiority, but the "everything must be compared to placebo" is a "no true Scotsman" fallacy.
