I was going to jokingly suggest someone make a tech startup dedicated to doing for keycaps and switches what AI and Crypto did to graphics cards and RAM, then I remembered the techie bumholes buying vintage tech to 'salvage' the vintage keyswitches from working retro computers and keyboards. The machine can't 'get it wrong' when you view civilian casualties as a bonus and as part of your wider goal. All the machine does is provide a figleaf to help give people who want to make excuses for genocide something to glom onto and point at when they claim the deaths weren't an intentional choice just down to flawed tech.

Who made and sold the tech to kill people knowing it was flawed?

A platform's success is now measured by the tech press in terms of exponential growth only. If you don't double or quadruple in size every year, you're doomed, dying or already dead. Stable isn't enough, even for things not designed to be for profit.
