There are some brain differences that show up in MRI's of people with schizophrenia as compared to people without it. Some people have it worse than others. I know 2 people with it. 1 was able to compensate without medicine for many years. Ocaisionally he would struggle to determine if something was real or not, and would ask somebody he trusted. In his early fifites, he lost this ability. The other person wasn't able to function without meds. She'd try but would become catatonic.
