New IBM PC book stuff!

There's still a lot of work left to do on these chapters, but I've committed them for now since it might be a little bit before I can come back to them.

There's a chapter on the Motorola MC6845:

I think the CGA chapter is shaping up to eventually be the authoritative source on the subject for all things CGA.

A treatment on composite artifact color is needed, but may ultimately be its own entire chapter.
