Well, over 20% of homes in the US use septic; so that's a hefty chunk that are not getting sewer from the government right there.
As to the market working well, competition is part of that. For years, I have challenged people to point out any significant monopoly that does not rely on government protection or assistance to survive. Not a single person has been able to do so.
