From my own experience, the issue is when someone starts believing their own thoughts are real, regardless of how unlikely they are, or even family/lived ones pleading they are not real.. This can quickly give the subconscious mind enough power to create auditory and visual hallucinations, that if also fully believed, continue to empower the subconscious. Snow balling into complete psychosis..
Brutal to watch a loved one go through this, for decades...

Years of tinnitus (shit sleep), causing feeling of constant exhaustion, and loneliness. Several siblings struggling financially + there relationships. Parents +80years, health and memory issues. A brother with gradually getting worse mental illness.
All make it hard for me not to just give up.. I could abandon all and live on the cheap on SE Asia.. That feels wrong though.
Have felt lost, all my life... Now, later in life it feels much more hopeless..
