Good question. There are actually two ratchets in MLS and we handle them differently.
For application messages, we don't coordinate generation picks across devices. We let them race. The delivery stream carries the generation in the clear, so when one device sends at gen N, the other sees it and advances its own counter past N. Self-echoes we match by ciphertext bytes (not leaf index), so a device knows if a "self" message was its own send or a sibling's, and advances accordingly. For the rare case where two devices grab the same N in the same instant, the RFC 9420 reuse guard adds fresh randomness to the nonce so the key/nonce pair stays distinct. That guard exists in single-device MLS anyway, so we get it for free. No fighting, no extra sync.
Commits are the part that can't be shared over the stream. A Commit refreshes the leaf with new keys, and only the committer gets the private half. A sibling trying to ingest it would mark itself removed. So we detect sibling Commits (sender leaf index equals ours), skip them, and the committing device republishes its full state on Blossom for the others to fast-forward from.
App messages ride the stream because their keys are derivable from the shared epoch secret. Commits can't, because the new leaf keys only exist on the committer.
