If you are referring to the individual patient in my thought experiment, not that long ago I would have said like yes whomever controls sensitive data should own their relay. While encryption exists, metadata could still leak, but we have NIPs now that address that, too, by gift-wrapping the metadata. Even right now with Nostr, a user doesn't have to run their own relay to own their data. A user can choose which apps can access it, which other relays it runs on, etc.
Note I don't consider myself technical and I MIGHT be wrong when in my interpretation of NIPs.
