I guess what's so brutal about government coercion is that it's institutionalized and uses the resources of the people to do its violence.
If someone grabs my purse on the street, I can at least try to fight back. When the government grabs my purse, I have to accept it or be willing to spend time in jail.
This applies to coercion in markets as well. A pharma company can make all sorts of horrible products and maybe the owners are liars and thieves - but it's not until the company makes a deal with the state that they can effectively steal everyone's money, brainwash nearly the entire population, and threaten almost everyone's livelihood for not using their product.
The state is what makes the theft and coercion systemic.
