Moderation and database, I’d say. Self-hosting for yourself doesn’t need moderation, but the moment you allow others to upload, you have a legal obligation to do it or face jail time. CSAM reporting obligations are also a hard one, and cannot just ignore it for the same reason. Lastly maintaining an always-on service can be a hustle, and ensuring tracking of security issues and such. I am sure I am forgetting a few more, but this is a good starter.

If you run an archival node, you sign up for 4 MB/block. Spam does not exceed this. This battle was resolved 9 years ago.
Also, there's not much reason for most people to run archival nodes these days. Pruned works just fine with lightning and electrum servers.
