Moderation and database, I’d say. Self-hosting for yourself doesn’t need moderation, but the moment you allow others to upload, you have a legal obligation to do it or face jail time. CSAM reporting obligations are also a hard one, and cannot just ignore it for the same reason. Lastly maintaining an always-on service can be a hustle, and ensuring tracking of security issues and such. I am sure I am forgetting a few more, but this is a good starter.
I guess to answer your "Why not" question, I suppose it's because the rest of the options appear to be less private than Phoenix, although I'm aware there are tradeoffs in there as well.
